Care Management and outreach services

CareSource has registered nurses, outreach workers and other health care professionals such as social workers on staff who can work with you one-on-one to help coordinate your health care needs. A CareSource Care Management professional can contact you by phone:

  • If your doctor requests one to contact you
  • If you request a call
  • If our Care Management staff feels our services would be beneficial to you

Upon initial contact and throughout the Care Management process, you may be asked questions to help us learn more about your condition(s). Our staff is trained to help you with any special medical or behavioral health problems you may have like diabetes or asthma. The Care Management staff will provide information to you to help you understand how to take care of yourself. We can also provide information on how to access services such as local community resources.

Our staff will talk with your PCP and other service providers to assist with coordination of care.

Our staff can also work with you if you need help figuring out when to get medical care from your PCP, urgent care center or emergency room.
