Where to get Care
Community Health Centers of Greater Dayton, Five Rivers Health Centers and other designated providers, provide primary care for Montgomery County Care members. Members of the program must choose a Primary Care Provider (PCP) who practices at these health centers. PCPs are your Medical Home providers. They perform yearly check-ups, diagnose medical conditions, prescribe medications, and refer you to specialists when necessary.
