South Community: $10,000 to help integrate primary and behavioral health care for young adults diagnosed with seriously mental illness and/or substance abuse issues

Suicide Prevention: $5,000 grant to fund “Project Lifesaver” K-12 suicide prevention education program for middle and high school students in a 24-county region

YMCA of Greater Dayton: $7,000 to impact childhood obesity in the African-American community including fitness training, nutrition education and partnership with GetUp Montgomery County

YWCA Dayton: $15,000 to provide emergency shelter for women and children who are survivors of domestic violence and/or child abuse

Family Outreach Center: $7,500 to support mental health services and support for low-income families in the Grand Rapids MI area

Food Banks of South Center Michigan: $5,000 to expand the “Kids After-School Backpack” program to supply weekend meals for over 1,500 low-income students

- ACCESS Shelter: $10,000 to provide emergency housing and self-sufficiency programs for homeless women and children in the Akron region
- American Red Cross, Dayton Chapter: $15,000 to provide humanitarian support for victims of disaster and those in need of temporary housing
- Assistance Dogs of America: $7,500 to provide specialty training for service dogs working with the elderly and people living with disabilities
- For Love of Children (FLOC): $3,000 to support gift distribution to more than 2,000 children in the Montgomery County foster or protective services
- Grandview Foundation: $15,000 to expand DO-WOP (Diabetes and Obesity Wellness Opportunities Program) collaborative designed to prevent childhood obesity in at-risk urban children
- Heritage Day Health Centers: $10,000 to provide adult daycare for almost 200 individuals who are frail, elderly disabled or low income in the Columbus region
- Hilltop Equestrian Center: $4,500 to support the equine-assisted physical therapy program for children with disabilities or life-threatening illness
- March of Dimes: $15,000 to help improved the health of babies by preventing birth defects, premature birth and infant mortality
- Mercy Neighborhood Ministries: $7,500 to provide visiting nurse services to promote health among low-income minority seniors in Cincinnati
- Metro Outreach Ministries: $5,000 to provide food and nutrition education for children in Montgomery & Clark County neighborhoods with high incidence of crimes, drug use, violence and poverty
- OUR Home Family Resource Center: $2,500 to support a central entry point through which residents of Mercer County can receive health and human services including food pantry, domestic violence shelter and court advocacy
- Rehabilitation Center for Neurological Development: $5,000 to partner with Miami County Children’s Services to pilot a Neurofeedback program targeting children with learning disabilities, anxiety, sleep disorders, migraine or brain injury
- Ronald McDonald House Cleveland: $5,000 to help provide home-like environment for families of critically ill children housed in Cleveland-area hospitals
- St. Vincent de Paul: $7,500 to address dramatic increases in the number of homeless families with children seeking emergency housing by supporting the Gateway Family Shelter
- YWCA Columbus: $9,000 to increase capacity in the family shelter and connect families with appropriate health/human services resources
- Access of West Michigan: $5,000 to reduce the impact of poverty in Kent County including expansion of the hunger response network and case management support services
- Alpha Women’s Center: $3,500 to support teen parent education and provide a safe daytime environment for homeless or at-risk mothers with babies
- Battle Creek YMCA: $3,500 for targeted anti-obesity campaign for families in the Calhoun County, Michigan region
- Cherry Street Health Services: $10,000 to support comprehensive primary and Preventative care in Kent County’s largest federally qualified health center with a focus on healthy kids
- Community Healthcare Connections: $5,000 for the Health Assistance Fund which provides medical transportation for low-income families when a child is hospitalized
