Care Management and outreach services
CareSource has nurses, outreach workers and other health care professionals such as social workers on staff who can work with you one-on-one to help coordinate your child's health care needs. A CareSource Care Management professional can contact you by phone:
- If your child's doctor requests one to contact you
- If you request a call
- If our Care Management staff feels its services would be beneficial to your child
Upon initial contact and throughout the Care Management process, you may be asked questions to help us learn more about your child's condition(s). Our staff is trained to help you with any special medical or behavioral health problems your child may have like cerebral palsy, chronic lung disease or cystic fibrosis. The Care Management staff will provide information to you to help you understand how to take care of your child. We can also provide information on how to access services such as local community resources.
Our staff will talk with your child's PCP and other service providers to assist with coordination of care.
Our staff can also work with you if you need help figuring out when to get medical care from your child's PCP, urgent care center or emergency room.

Preventive medical care is an important part of keeping your child healthy. Regular care helps your child's PCP find problems early so they can be treated before they get worse.
