The Community Health Centers of Greater Dayton provide primary care for Montgomery County Care members. Members of the Montgomery County Care program must choose a Primary Care Physician who practices at these health centers.

Your PCP is your Medical Home provider. Primary Care providers perform yearly check-ups, diagnose condition prescribe medications, and refer you to specialists when necessary.

Provide high-quality affordable health care for uninsured and insured. If you move out of the area or are visiting outside of Montgomery County, the Ohio Association of Community Health Centers can assist you in finding a health center where you can receive affordable medical care.

Offer free health care for uninsured. Prescriptions are filled through patient assistance programs, drug repositories, purchased medications and physician samples. If you move out of the area or are visiting outside of Montgomery County, the Ohio Association of Community Health Centers can assist you in finding a health center where you can receive affordable medical care.

Local dental and vision care
The Montgomery County Care program does not cover the cost of dental or vision care. However, there are several resources in the area that offer affordable care. The Community Health Centers of Greater Dayton can also assist you in finding these services.

Financial assistance with prescriptions
Some drugs prescribed by your doctor may not be covered by the Montgomery County Care program. Assistance may be available to help cover the costs through other programs.
Ohio’s Best Rx Program

This program is the result of legislation passed by the Ohio General Assembly and signed by the Governor in December 2003. The program is a prescription drug discount card program designed to lower the cost of prescriptions for Ohio residents without drug insurance coverage who are either aged 60 and over or any age with incomes less than 300% of the federal poverty level.

Each state is required by federal law to have a Disproportionate Share Hospital (DSH) program. These programs are intended to ensure that uninsured individuals have access to hospital care. Ohio’s DSH program is called the Hospital Care Assurance Program (HCAP). HCAP provides a payment to hospitals for providing uncompensated or charity care to low-income and uninsured individuals. The hospitals will help you apply if you call and ask them about HCAP.
