How to Enroll
You must complete an application form and be approved to be enrolled in Montgomery County Care. Please complete the Request for Enrollment Information Form if you would like to receive enrollment materials.
You can also call Member Services at 1-877-892-7482 (TTY: 1-800-750-0750 or 711) with questions about the Montgomery County Care program.
Please note: Montgomery County Care is a pilot program covering a limited number of enrollees. You may qualify for the program, but you may be put on a waiting list if the program is not accepting new members at that time.
How will I find out if my application has been approved?
If your application is approved, CareSource will send you a letter confirming your coverage start date. CareSource will then mail you a member ID card as well as a set of new member materials.
How long am I enrolled in the program?
If your application is approved, you will remain eligible for coverage for 12 months from your coverage start date. You will need to re-apply for enrollment in the Montgomery County Care program every year.
Changes in your household income or adding other medical coverage may affect your eligibility for the Montgomery County Care program. Approved members must let CareSource know if there are any changes:
- To your household size
- To your income
- To your medical coverage
