CareSource recently announced the launch of its new Member Profile, an innovative, web-based service that provides physicians and other healthcare providers with easy-to access, comprehensive patient historical information designed to improve patient quality and outcomes. The Member Profile is designed to pull from a variety of data sources and presents a complete view of recent utilization, clinical quality and medication history. This represents a major step forward in effectively managing the healthcare of CareSource's primarily Medicaid population.

With its comprehensive view of patient medical and pharmacy data, CareSource’s Member Profile can help you improve your CareSource patients’ health outcomes. The profile can help you determine an accurate diagnosis more efficiently, reduce unnecessary diagnostic tests, and minimize ER visits. Accessing the Member Profile feature is fast and easy.  Participating providers will find the Member Profile within the CareSource Provider Portal.

CareSource Clinical Practice Registry

The CareSource Clinical Practice Registry is available online. Quick and easy to access on our secure online Provider Portal, the Registry includes population and member-level actionable and relevant information. You'll find information on: 

  • Asthma
  • Breast Cancer Screening
  • Cervical Cancer Screening
  • Chlamydia Screening
  • Diabetes
  • Emergency Room Visits
  • Lead Screening
  • Well Baby Visits (0 to 15 months old)
  • Well Care (2 to 21 years old)

The report is color coded and is intended to encourage you to contact CareSource members for preventive visits, flag charts for needed services, or simply create an awareness of the patient's need for an intervention. And for your convenience, the report can be downloaded in PDF or Excel spreadsheet format. It also provides accessibility to the CareSource Member Profile for individual members of interest.
