How to Apply
The CareSource Foundation funds programs that improve health outcomes and conditions for low-income, underserved populations in the communities where we live and work. We are especially interested in efforts that will foster learning and innovation.
We invest in:
- Health education, disease management and prevention programs that meaningfully improve physical and mental health for targeted populations.
- Programs that promote healthy births and enable vulnerable newborns, children and parents to thrive.
- Transformative programs and partnerships that help revive and rebuild healthy, thriving neighborhoods, reduce health inequities, and create economic opportunity for vulnerable populations.
- Effective P-20 education programs and initiatives that cultivate and prepare the next generation of professionals, and foster a high-quality, diverse workforce.
Additionally, we are engaging in partnerships with local and national community nonprofits that align with our areas of focus.
The CareSource Foundation provides two types of grants:
- Signature Grants are strategic, targeted programs that produce a long-term, large-scale impact conducted over a longer period of time. Depending on the size of the grant request, it may require approval by the CareSource Foundation Board of Trustees. Please allow adequate time between the time you apply and when a decision will be made.
- Community Grants are responsive grants strategically aligned with our focus areas and have a direct impact on the local needs of the states we serve. These grants are smaller, awarded year-round by CareSource Foundation committees established in each market.
All grants requests must be made through our online grant application system. The foundation will not accept proposals via email or in other formats. Additional documents may be attached to the application in the online system.
To quality for grant funding, organizations must be tax-exempt under the IRS Code Section 501(c)(3). The foundation prefers to provide funding to an organization once in a 12-month period. This does not preclude you from applying for more than one program during a 12-month period, but receiving multiple grants during the same period is rare.
The foundation primarily supports programs within the states CareSource does business. Currently, these states are:
- West Virginia
As CareSource’s footprint expands, we will consider requests from additional locations on a case-by-case business.
APPLICATION DUE DATES
We accept applications year-round. However, to better guide expectations for a timely review and response to your application, the Foundation Board and Review Committees follow the anticipated schedule below.

Community Grants are reviewed on an on-going basis. Applications received by the first business day of each month will be reviewed by our committees the following month (i.e. applications received by February 1 are reviewed during the March committee meeting). Committee meetings generally occur the second week of each month; however, all schedules are subject to change.
Please keep in mind that even if your request is submitted within the timeframes above, missing information or documents may affect the staff’s ability to process your grant request.
IMPORTANT: The online application will not be available between the second business day of December and December 31 for maintenance. All applications not submitted before the second business day of December will not be accepted and will require a new application to be started after January 1.
KEY APPLICATION ELEMENTS
Program activities should provide details about what will occur during the grant period, including activities planned, their frequency and duration, and identify participants or beneficiaries. A detailed description of each program component serves not only to delineate activities intended to occur during a program year, it also provides context for assessing the program’s implementation at year-end.
Before implementing a program, an organization should outline measurable short-term outcomes that will demonstrate the change expected to occur as a result of the program activities. This type of information provides important benchmarks for determining if a program is creating the desired results that will lead to its ultimate goal.
The long-term impact of a project is reflected in the cumulative or lasting changes it effects. Establishing tangible, long-term goals and identifying indicators of success helps to guide the direction of a program and provides a framework for assessing its effectiveness over time.
Before you apply, ensure you have the following documents:
- IRS 501(c)3 determination letter
- Current W-9 Form
- Current Board of Directors List
- Organization’s operating financial statements (balance sheet & statement of activities) for current fiscal year (year-to-date and month-to-date)
- Program Budget for program period
- Most recent audited financial statements. A 990 is acceptable only if audit is unavailable and your gross revenue is less than $500,000
- Current Annual Report
- Other Documentation
Upon completion of grants, a final report that outlines program outcomes will be required. Depending on the grant amount and grant length, an interim report on any significant accomplishments or program changes may also be required. Final reports to the Foundation should compare and contrast the actual program activities, short-term outcomes, and long-term impact with the projections offered in the application. Grantees who have not completed a final grant report will not be eligible for future funding.
