We want you to easily find the documents you need for your CareSource PASSE plan. Explanations of when and why you may need to use a document are also provided below.
- Member Handbook: The member handbook is a quick reference guide to your health care benefits. You can quickly look up information such as your covered benefits and services, where to get care and your rights and responsibilities as a CareSource PASSE member.
- New Member Booklet: The new member booklet is mailed to all new CareSource PASSE members. It has a high-level overview of benefits and services, and the printed version that is mailed also has the CareSource PASSE member ID card.
- Non-Discrimination Notice: This notice goes over how to get help and information in your preferred language and how to get special help if you need special help communicating to CareSource PASSE or providers.
- Cultural Competency Plan (coming soon): This document goes over how CareSource PASSE recognizes cultural needs and preferences and how we serve our diverse constituents using a culturally sensitive approach.
