The TRICARE Pharmacy Program and Formulary are managed by the pharmacy contractor, Express Scripts. Please visit the TRICARE Covered Services > Pharmacy > Prescription Drugs page or use the TRICARE Formulary Tool to locate prescription drugs that we cover. Pharmacy Prior Authorization Medical Drug Prior Authorization Form (coming soon): Submit this form to request a prior authorization to prescribe provider administered drugs covered and reimbursable by CSMV under the medical benefit. Please see the Pharmacy page for more information.
