Select Primary Provider

Want to choose or change your primary care provider (PCP)?  Log into your My CareSource^® account. Choose one of the providers in the network. You can also call us.

Your PCP is listed on your ID card. You will get a new ID card  when you choose a new provider. Your new card should  be in the mail within 10 days.

 You can also choose your Dental Home. Log into your My CareSource^® account. Choose a dentist. You can also call us.

Your dentist is also listed on your ID card. You will get a new ID card when you choose a new dentist. Your new card should be in the mail within 10 days.

Use the CareSource mobile app for a digital image of your ID card. The mobile app is available for both iPhone and Android. Get it free through the Apple App Store^® or Google Play^® today.
