Health Risk Screening

We want to help you stay healthy. When you fill out your Health Risk Screening (HRS) it helps us understand your health needs and make sure you’re getting the care you need.

You can complete the HRS in one of these ways:

• Online: Go to MyLife.MSTrueCare.com. Sign up or log in to fill out your HRS.
• Mail: Complete the printed copy of the HRS. You will get this in the mail after you get your new member booklet. Return it in the enclosed self-addressed postage paid envelope.
• Phone: Quickest and easiest way. Call 1-844-542-2610 to take the health survey over the phone. Our staff will help you Monday through Friday between 7 a.m. to 6 p.m. Eastern Time.
