My CareSource Rewards Program
Get rewarded for healthy activities with the My CareSource Rewards program.
How does it work?
- Get your Healthy Benefits+ card.
All members are automatically enrolled. You will get the Healthy Benefits+ card in the mail when you join the plan. This is the same card you use for your Healthy Benefits+ monthly allowance.
- Complete healthy activities.
Rewards are added to your card when you complete healthy activities. It can take up to 60 days from the time we get the claim for your reward to show on your card. Learn what activities you can do to earn rewards below.
- Redeem your rewards.

What rewards can I earn?
Rewards are earned once each year unless noted.
Rewards for Immunizations
| $50 | Flu shot. Earn $50 for each shot up to two times each year. |
Rewards for Preventive Care
| $50 | Annual physical exam. |
| $10 | Annual dental exam. |
| $10 | Sign up and link your plan in CareSource MyLife. |
| $50 | Colorectal cancer screening. |
| $50 | A1C test. Earn $50 for each test up to two times each year. Must have a diabetes diagnosis. |
| $25 | Kidney health evaluation. Must have a diabetes diagnosis. |
| $50 | Retinal eye exam with an eye care provider. Must have a diabetes diagnosis. |
| $50 | Breast cancer screening (mammogram). For women ages 40 and older. |
| $10 | Follow up visit for Osteoporosis after a diagnosed fracture. For those ages 67 and older. |
| $50 | Engagement with your Care Coordinator after a hospital discharge. You must engage within 30 days of the discharge. |
| $50 | Complete your Health Needs Assessment. Earn $50 up to two times each year. Must be six months or longer between each assessment. |
Rewards for Expectant Moms
| $85 | Prenatal visits. You must complete seven prenatal visits to earn this reward. Earn once each pregnancy. |
| $25 | Postpartum visit. Earn once each pregnancy. Must have the visit 21 – 56 days after delivery. |

Rewards are subject to change. Rewards may vary by age, gender, and health needs. Rewards may expire based on date of issuance. If you are no longer a CareSource member, your access to the rewards portal will be deactivated, but the reward amount will remain on the card.
