Macomb & Wayne County
You are eligible for our plan if the following are true:
- You have Medicare Part A, Part B & Part D
- You are eligible for full Michigan Medicaid who live in our service area
- You are a United States citizen or are lawfully present in the United States
- You are not already enrolled in hospice
- You are not enrolled in the MI Choice waiver program or the Program All-Inclusive Care for the Elderly (PACE). If you are enrolled in either of these programs, you need to disenroll before enrolling in the MI Health Link program through HAP CareSource MI Health Link.
To enroll, call the HAP CareSource MI Health Link Customer Service at 1-833-230-2057 (TTY: 1-833-711-4711 or 711), or visit HAPCareSource.com and select HAP CareSource MI Health Link. We are open 8 a.m. to 8 p.m., Monday through Friday.

Over-the-Counter (OTC) Benefits
Receive a quarterly OTC benefit online, by phone or by mail. You can get over-the-counter (OTC) items for allergy relief, vitamins, OTC pain relievers, blood pressure monitors and more.
HAP CareSource MI Health Link rewards its members for taking a proactive role in their health and well-being. We have developed a lifestyle rewards program to encourage you to participate in various annual wellness visits and preventive care programs.
HAP CareSource MI Health Link Care Coordination works with you to help you achieve what matters most. Your Care Coordinator will work with you to develop a person-centered support plan and partner with you in your health care journey to ensure we support your physical, mental and social needs.
Any changes in phone number, email, or address should be reported to the Michigan Department of Health and Human Services (MDHHS). You can do this by going to the MIBridges website at www.michigan.gov/mibridges. If you do not have an account, you will need to create an account by selecting “Register”. Once in your account, when reporting changes, please make sure you do so in both the profile section and the Report Changes area. The Report Changes area is what the local office will use to update the address for your case.
HAP CareSource™ MI Health Link (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Michigan Medicaid to provide of both programs to enrollees.
