Are you currently paying for your own phone? As a CareSource HIP or HHW plan member, you qualify for a free smartphone. This includes:

  • free text
  • 1,000 talk minutes per month
  • 5 GB of data

How to Get Your Free Smartphone

Applying only takes a few minutes. Go to https://mybenefitphone.com/ and follow these steps:

  1. Enter your Date of Birth and Medicaid ID. Your Medicaid ID is your member ID number. You can find this on your ID card. Then hit the “start application” button.
  2. Follow the application question prompts. Confirm that all the information listed is correct. Make any changes if needed.
  3. Agree to all the terms and conditions of the program, including the terms and conditions on the National Verifier part of the application.
  4. Finally, submit the completed form.

Once you are approved, your new phone will be shipped to you.
