Don’t Risk Losing Your CareSource Health Care Coverage.
CareSource cares about you. Our mission is to help you stay healthy. Beginning in April 2023, you or your child may need to take action to keep your coverage. To stay a CareSource member, you have to renew your benefits. You can renew in one of the following ways:
- Online. Login to your FSSA Benefits Portal at fssabenefits.in.gov to complete your renewal online.
- By Mail. Fill out the renewal forms. Please include any additional information the state has requested.
- In Person. Visit your local Division of Family Resources (DFR). Find the office nearest to you at https://www.in.gov/fssa/dfr/ebt-hoosier-works-card/find-my-local-dfr-office/. Turn in your completed renewal form and any other requested information at your local DFR.
The state will send you a renewal notice when it is time to renew your Healthy Indiana Plan or Hoosier Healthwise coverage. Please make sure that FSSA and CareSource have your current address and phone number. Watch your mail for reminders from CareSource and your renewal form from FSSA. You could lose your eligibility for health care coverage and your extra CareSource benefits.

CareSource cannot renew your benefits. It must be handled by your local DFR office. However, if you need a ride to your local county DFR office, CareSource can help. To schedule a ride, please call 1-844-607-2829 (TTY: 1-800-743-3333 or 711) at least two business days before you need a ride.
