We want you to easily find the documents you need for your CareSource PASSE plan. 

Documents

• Navigate Member Handbook: This is a guide to your benefits. Look  up your covered services, where to get care, and your rights and responsibilities as a member.
• Navigate New Member Booklet: This is mailed to you when you enroll. It has an overview of your plan. It also comes with your member ID card.
• Provider Directory: The most up-to-date list of providers is available through our Find a Doctor tool. This tool will show you:
  • Primary care providers (PCPs)
  • Primary dental providers (PDPs)/Dental Home
  • Specialists
  • Hospitalists

• Navigate Non-Discrimination Notice: Learn how to get help in your preferred language.  You can also get materials in formats like large print, braille, or audio.
• Navigate Cultural Competency Plan: This explains how we recognize cultural needs and preferences. We use a culturally sensitive approach to serve you.
