How to Join the CareSource Bayou Health Network
CareSource Bayou Health LLC responded to the Texas Health and Human Services Commission request for proposals, for an opportunity to serve members in Harris Service Area and Jefferson Service Area who are part of the State of Texas Access Reform (STAR) Program and Children’s Health Insurance Program (CHIP). We invite you to join our network as a participating provider with CareSource Bayou Health.

Already Contracted with Us?
Thank you for your interest in joining CareSource Bayou Health LLC (CareSource Bayou Health)! We are in the process of contracting a provider network in Texas. We appreciate your commitment to being a part of the CareSource Bayou Health network during this development period. If you have already submitted a contract, no further action on your part is required.
Upon CareSource Bayou Health receiving a contract from the Texas Health and Human Services Commission, we will start the process of application and credentialing your practice using guidelines from the state and National Committee for Quality Assurance (NCQA). Once the credentialing process is completed and your practice is loaded into our internal systems, you will receive a welcome packet that includes a Welcome Letter with your CareSource ID number, and a fully executed copy of your provider participation agreement. You will then be viewable in our online Find a Doctor provider directory. If you have any further questions, please feel free to contact us!
