CareSource is an HMO D-SNP with a Medicare and state Medicaid contract. Enrollment in CareSource depends on contract renewal.

CareSource offers the CareSource Dual Advantage™  (HMO D-SNP) and CareSource Dual Advantage™  Plus (HMO-DSNP) plan, which vary by prescription drug coverage.

***$0 copay covered medical benefits include primary care provider (PCP)/specialist office visits, inpatient hospital care, emergency room (ER), urgent care visits, preventative care, routine podiatry, annual physical, home health care, ambulance services, durable medical equipment (DME), telehealth or behavioral health visit, and personal emergency response system.

**** Rewards are subject to change. Rewards may vary by age, gender, and health needs.  Rewards may expire based on date of issuance. If you are no longer a CareSource member, your access to the rewards portal will be deactivated, but the reward amount will remain on the card.
