Will you share your thoughts and experiences by taking a short survey?
Do you have a few minutes to take a survey and share about your experience with the services you receive? The Ohio Department of Medicaid (ODM) is partnering with Public Consulting Group, (PCG) to improve Home- and Community-Based services (HCBS) for individuals in Ohio. Your feedback is critical in helping us better understand your experiences.
All responses will be kept confidential and anonymous. Also, your participation will not affect your services or Medicaid benefits in any way.

If you are unable to complete the survey online, you may complete the survey over the phone by calling PCG’s call center at 1-877-908-1746 during our normal operating hours, Monday-Friday, 8 a.m. – 4:30 p.m. ET. A member of PCG’s call center team will be glad to help you.
