Important Announcement about CareSource Medicare Advantage:

Your Medicare plan won’t be offered in Kentucky in 2018.

You should have received a letter from CareSource letting you know that we won’t offer your Medicare plan in 2018. This means health care coverage through CareSource Advantage® Zero Premium (HMO), CareSource Advantage® (HMO) and CareSource Advantage Plus® (HMO) in Kentucky will end December 31, 2017.

Take action by December 31, 2017.

You should choose and join a Medicare plan before December 31, 2017 to make sure you have coverage on January 1, 2018. If you do not take action before December 31, 2017, you will lose prescription drug coverage and only be covered by Original Medicare starting January 1, 2018. Because CareSource Advantage will no longer be offered, you can join a new plan anytime between October 15, 2017 and February 28, 2018. If you don’t join a plan with prescription drug coverage by February 28, 2018, you won’t have prescription drug coverage in 2018 and you may have to pay a lifetime Part D late-enrollment penalty if you join a Medicare prescription drug plan later.

Compare your choices for a new Medicare plan.

You need to review your options and choose what is best for you. You may want to join another Medicare health plan or you may decide to change to Original Medicare. If you choose Original Medicare, you need to join a separate Medicare prescription drug plan to get prescription drug coverage. You may also want to buy a Medicare Supplement Insurance (Medigap) policy to fill in the gaps in Original Medicare coverage.

Don’t wait - get help now.

It’s important to find a plan that covers your doctor and prescription drugs. Refer to your Medicare & You Handbook for a list of all Medicare health and prescription drug plans in your area. You can get help comparing plans if you:
