Congratulations on your recent release and participation in the Community Transition Program!
CareSource is partnering with the Ohio Department of Mental Health and Addiction Services (OhioMHAS) to support your continued recovery as you return home. Our goal is to improve your health and well-being with a plan to help you achieve your goals.
Treatment and Recovery Services
We recognize that good health means more than just taking care of your physical needs. That is why the Community Transition Program helps you with the following services.

Please apply for Medicaid immediately as this is a transitional benefit that does not include comprehensive health coverage. The treatment services listed above are covered by Ohio Medicaid. You should immediately enroll in Medicaid to ensure ongoing coverage of these services. You are not required to choose CareSource as your Medicaid plan. You may contact your CTP provider to assist with Medicaid enrollment.

Personalized Care Coordination
You received an ID card and a personalized reentry service plan when you enrolled in CTP. Your service plan includes the CTP provider who is assigned to work with you. Be sure to contact this provider and start working on your goals. You will need your ID card to get access to resources in your community.

How to Enroll in the Community Transition Program
If you think you might be eligible for the Community Transition Program but are not yet enrolled, contact your Parole Officer, your Community Mental Health Center Case Manager or the Ohio Department of Mental Health and Addiction Services at 1-877-275-6364 or www.mha.ohio.gov/.
