Find My Prescriptions

Use the Find My Prescriptions tool and Drug List to verify coverage and check the costs of drugs and some supplies for HAP CareSource MI Coordinated Health.

If you are a member, your plan name is on your HAP CareSource MI Coordinated Health member ID card. Members and health partners may be contacted by a pharmacist to discuss medications. We encourage members to talk with their pharmacists about their medications. This can help members to get the best results from the drugs they are taking.

HAP CareSource MI Coordinated Health uses a List of Covered Drugs (Formulary) (coming soon). The list shows the drugs we like our providers to prescribe. The List of Covered Drugs (Formulary) is the entire list of Part D drugs covered by our Part D plan. This includes the Part D drugs covered by Medicare and the non-Part D and over-the-counter (OTC) products covered by Medicaid.
