Use the Find A Pharmacy tool and List of Covered Drugs to check coverage. You can also login into your CareSource MyLife account to see your pharmacy benefits.
List of Covered Drugs (Formulary)
The List of Covered Drugs (Formulary) lists both the Medicare Part D and Medicaid drugs covered under your plan. Some drugs have restrictions and/or limits. This includes prior authorization, step therapy, and quantity limits. Changes to drugs on our list will be posted on the Notice of Formulary Changes.
We will give a 30-day notice to you when:
- Taking a drug off the list.
- Changing a prior authorization.
- Changing quantity limits.
- Changing cost tiers.
