End or Change Membership

We want you to be happy with us. If you are not, please let us know – we want to make it right.

You can request to change or end your membership in HAP CareSource™ MI Coordinated Health at any time by choosing a different plan.

Choosing a Different MI Coordinated Health Plan

If you want to keep getting your Medicare and Michigan Medicaid benefits together from a single plan, you can join a different MI Coordinated Health Plan.

Call Michigan ENROLLS at 1-800-975-7630 (TTY: 1-888-263-5897) Monday through Friday, 8 a.m. to 7 p.m. Eastern Time. Tell them you want to leave HAP CareSource MI Coordinated Health and join a different MI Coordinated Health Plan. If you are not sure what plan you want to join, they can tell you about other plans in your area.

Your coverage with HAP CareSource MI Coordinated Health will end on the last day of the month that we get your request.

When We Must End Your Membership 

We must end your membership if any of the following happen:

  • If you do not stay eligible and enrolled in Medicare and Medicaid. You must renew your Medicaid benefits each year to keep your benefits.
  • If you move out of our service area.

We may not end your membership for any reason related to your health.
