Who Can Participate in the HAP CareSource JobConnect Program?

HAP CareSource JobConnect serves members that are:

  • HAP CareSource members or parents/legal guardians of a minor child who is a HAP CareSource member
  • Over the age of 14
  • Interested in pursuing a goal that improves the member’s health or overall well-being

What Should I do Next If I Have Interest?

Since HAP CareSource JobConnect is a voluntary program, you will need to let us know you are interested. You may contact us by:

Next, we will contact you to set up a time to learn more about you and your needs.

If you decide to participate, you will need to fill out and sign forms that will give us permission to work with you. Based on your goals and what barriers you face, we work together to set up an easy-to-follow long term plan to help you succeed. Members are assigned a Life Coach who will continue to work with you as you work toward your goals. The support provided by the Life Coach varies based on what each member wants and needs.
