Don’t Lose Your Coverage
We love that you are a member of HAP CareSource MI Coordinated Health! Medicaid eligibility reviews happen annually. Most people need to renew their Medicaid coverage each year so that they can keep their health coverage. We cannot renew for you. You need to renew through your local Michigan Department of Health & Human Services (MDHHS) office.
When it is time to renew, MDHHS will send a renewal form when it is time to renew. your Medicaid coverage. If you don’t take action, you could lose your Medicaid eligibility. Without Medicaid coverage, you will not be eligible for HAP CareSource MI Coordinated Health.
There are three easy steps you need to take to help determine if you still qualify for coverage:
- Update your contact information at Michigan.gov/MIBridges.
- Check your mail or text messages for a notification letter from MDHHS.
- Complete your MDHHS renewal form if you get one.
Visit MI Bridges or contact your local MDHHS office if you need help. If you don’t have a MIBridges account, you will need to create one. Once you’re in your own account, make sure to put all of your information in correctly. If you have to go back in and change something, put the changes in both the Profile and Report Changes sections.
Before you submit your Medicaid renewal, be sure to have the following documentation that applies to you handy. You need proof of:

After you send in your renewal, you may receive a Verification Checklist if any documents are missing. Be sure to review and respond to this request.
