Programs for your health and well-being

At HAP CareSource, you’re not just an ID number. You’re a valuable member of our family. And we want our family to be happy, healthy and strong.

As a member, you have access to health and wellness services. Our wellness programs help you make good changes to your health.

Mom and Baby Beginnings Program

The Mom and Baby Beginnings Program is here to help you during your pregnancy. We have a team of nurses, social workers, counselors and breastfeeding experts who have been working with members just like you for many years. Our team will help you get connected to resources in your community that can help you with your pregnancy, food, housing, transportation and safety. We will also collaborate with your medical team to make sure you are healthy and safe during your pregnancy. The experts on our team will make sure you have the answers you need to understand your pregnancy and how to take care of your newborn.

Health Rewards Program

Your health and well-being are important to us. To help you stay healthy, we’ll reward you for checkups and screenings. You must be a HAP CareSource member on the date of service to get rewards.

Easy Access to Care

24-Hour Nurse Advice Line

HAP CareSource members can use a free 24/7 health information line.

Nurses answer your questions.

It gives:

  • Answers to health questions
  • Trusted information

When you call, a nurse helps you:

  • Know where to get care.
  • Understand your condition.
  • Be healthy.
  • Take medications safely.

Telehealth

Telehealth is convenient option for care. Instead of going into an office for your appointment, you can use your phone or computer to see and talk to your primary care provider (PCP) or other health care provider from wherever you are. There is no cost to you and telehealth removes the stress of needing transportation services.

As a HAP CareSource member, you have free rides to and from medical appointments. This includes:

  • Doctor
  • Dentist
  • Eye doctor
  • Hearing
  • Pharmacy

Care Support

Care Management

This program helps you coordinate your care and guide you to the health care services you may need. It’s for members with a critical event or diagnosis that needs:

  • Extensive and many different resources
  • Special care and services
  • Education on chronic conditions, help in finding care, coordination of care

HIV Resources

Have you or someone in your household been diagnosed with HIV? If so, you can connect with resources to support your care. Please visit Michigan.gov/HIVSTI for a list of resources. You can also call our Care Management team at 1-844-217-1357 (TTY: 711) for help.

Help to stop smoking

The Michigan Tobacco Quitline is a free phone-based program to help you quit smoking. You’ll work with a health coach to make a quit plan. Your coach will call to make sure you’re on track. To sign up, call 1-800-QUIT-NOW (784-8669). To learn more, call 1-833-230-2053 (TTY: 711).

My CareSource® Member Portal

My CareSource is a secure account where you can find out about your benefits, see plan records, and make changes to your care. My CareSource accounts can be linked to manage health care for families.

Mobile App

Manage your HAP CareSource plan on the go with the CareSource mobile app. The app lets you access your secure My CareSource account from your mobile device. The CareSource mobile app is available for both iPhone and Android systems. Get it free through the App Store® or Google Play®.
