Get support tailored to your unique needs. We are a single point of contact working with you and your providers to coordinate your care. Our team can help you navigate barriers like:
- Helping get your medications and supplies.
- Explaining insurance benefits and services.
- Providing education on chronic illnesses.
- Connecting to community resources.
- Helping you coordinate transportation.
- Finding after-hours support if you need it.
Care management has different levels of care. Your level of care depends on your condition and how much help you need. One of the easiest ways to get started is by completing your Health Needs Assessment.

Disease Management
If you are living with chronic health issues our disease management programs are just for you. You can choose to be part of a program or we may hear from your doctor, pharmacy or other provider that you would benefit from this program. We want to help you be healthy and well.

Better Choices, Better Health
Are you or a loved one in your household living with a long-term health condition? You can get help staying healthy without leaving your home. When you join Better Choices, Better Health® (BCBH), you’ll join a small group of people just like you managing pain, stress and increased isolation. The 6-week online workshop is free to adults who have a chronic health condition or disability. BCBH is proven to help people:
- Better manage chronic pain and stress
- Reduce feelings of loneliness and depression\
- Lower blood sugar and increase physical activity
