Foster Care

Our team helps children and youth in foster care. We support kids, their caregivers, and foster agencies. You will get a Care Manager to help you understand your health care needs. We can help you:

• Secure important documents.
• Find education support.
• Find employment and resources.
• Secure housing.

Health Care Transition

The Health Care Transition (HCT) program helps those aging out of foster care with their health care needs as they enter adulthood. Your Care Manager can:

• Help with the Transition Readiness Assessment Tool. This tool shows areas where you may need extra help on your health care transition.
• Teach you about the privacy and consent changes when you turn 18.
• Help you find adult providers.
