Eligibility
You are eligible for our plan if the following are true:
- You are aged 21 or older and live in our service area.
- You have Medicare Part A, Part B and Part D.
- You receive full Medicaid benefits including those with a 1915(c) waiver or who live in a Nursing Facility, along with those who have a monthly Patient Pay Amount.
- You do not live in a state-operated veteran’s home.
- You are not currently enrolled in hospice care.

Call Michigan ENROLLS at 1-800-975-7630 (TTY: 1-888-263-5897). They are open Monday through Friday (except holidays) 8 a.m. to 7 p.m. Eastern Time. Wait times are usually shorter before 10 a.m., after 2 p.m., and later in the week.

Care Coordination
We will work with you to help you achieve what matters most. Your Care Coordinator will work with you to develop a person-centered support plan and partner with you in your health care journey to ensure we support your physical, mental and social needs.
Any changes in phone number, email, or address should be reported to the Michigan Department of Health and Human Services (MDHHS). You can do this by going to the MIBridges website at www.michigan.gov/mibridges. If you do not have an account, you will need to create an account by selecting “Register”. Once in your account, when reporting changes, please make sure you do so in both the profile section and the Report Changes area. The Report Changes area is what the local office will use to update the address for your case.
HAP CareSource™ MI Health Link (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Michigan Medicaid to provide of both programs to enrollees.
