HAP CareSource™ MI Coordinated Health (HMO D-SNP) offers Care Coordination that can provide a broad spectrum of educational and follow-up services for your patients.

You can access care plans online through the provider portal to easily provide input into your patients’ care plans and consult with care coordinators.

Care Coordination Program

HAP CareSource MI Coordinated Health’s Care Coordination program is a fully integrated health management program that strives for member understanding of and satisfaction with their medical care.

Our one-on-one personal interaction with outreach specialists and nurse care coordinators helps support your patient through the following:

  • Initial and ongoing assessment activities
  • Coordination of care
  • Education to promote self-management
  • Providing additional community resources
  • Transportation assistance

We offer individualized education and support for many conditions and needs, including:

Asthma

Diabetes

Heart Disease

Depression

High Blood Pressure and Cholesterol

Low Back Pain

Pregnancy

Weight Loss

Disease Management Programs

Members identified as high risk will have a nurse assigned to their case to help educate, coordinate and provide resources and tools to help the member reach their health care goals.

Our Disease Management programs help our members find a path to better health through information, resources and support.

We help our members through:

  • The MyHealth online program for members 21+ to participate in a journey to improve their health
  • Access to health education and an Interactive Health Library within the MyLife app which has helpful tips and information
  • Coordination with outreach teams
  • One-to-one care coordination (if they qualify)

Members with specific disease conditions are identified by criteria or triggers such as emergency room visits, hospital admissions and health assessment. All ages (children, teens, and adults) are eligible. These members receive notifications via text, email or letter when new educational materials are available in their MyLife App to view, prompting them to sign in and review the digital content. Any member may self-refer or be referred into the Disease Management Programs to receive condition-specific information or outreach.

Benefits to Members and Health Partners

Members identified in the Disease Management programs receive help finding the appropriate level of care for their condition, and they are encouraged to actively participate in the patient-provider relationship. The programs improve the percentage of members who receive their recommended screenings.
