Keeping your provider information up to date is crucial for timely claims processing and enhancing patient experiences. Accurate data improves patient access, allowing them to easily find and connect with you, which leads to better care coordination and satisfaction.
Update Your Information
Providers should continue to submit provider adds, demographic or status changes and terminations via the HAP CareSource™ Provider Change Form.
Once complete, email the form to providernetwork@hap.org with the subject line: Provider Change Form. In the body of the email, be sure to include:
Type 1 NPI
Type 2 NPI
Tax ID
Please be sure your office address, phone, fax, etc. are up to date in the National Plan & Provider Enumeration System or NPPES. Pharmacy benefit managers typically use DEA and NPPES systems to send required patient-level notices, such as transition letters and approval or denial letters.
