Drug Formulary

HAP CareSource™ aligns with the state and all Michigan Medicaid Health Plans and administers the Common Formulary/Single Preferred Drug List (SPDL). The list is updated at least four times a year and includes all covered drugs. More information, including the Prior Authorization Criteria, may be found here.

Online Drug Formulary

We offer an online Formulary Search Tool that can help you quickly look up medications for HAP CareSource members. It’s easy! You can find out if a medication is covered by searching alphabetically, by brand and generic name, or by therapeutic class.

Drug Formulary Changes

Each quarter, there may be changes to the Common Formulary/Single Preferred Drug List. You can check the list of drug formulary changes to see if any of these changes affect the medications you prescribe.
