Caregiving.org

Technology, training and caring for a loved one can elevate stress levels, lead to social isolation, and cause physical health challenges for the caregiver. We support these unsung heroes with a range of evidence-informed technology solutions and training and education opportunities, consistent with Schedule A SOW, Section 6.10.

This includes:

  1. self-care and preventing compassion fatigue,
  2. caring for an Enrollee with disabilities, mental illness, or cognitive impairment,
  3. recognizing signs of abuse and neglect,
  4. how to administer medications and injections,
  5. ensuring safe Enrollee physical transfers,
  6. precautions to prevent infectious diseases in the home and community,
  7. understanding the Enrollee’s illness, condition, or disability,
  8. appreciating the significance of an informal caregiver’s role in the overall health and welfare of the Enrollee,
  9. how to identify and report suspected abuse and neglect, and
  10. post-discharge care at home, including disease-specific education and resources.

Care Coordinators link caregivers and informal caregivers to CaregiverConnect, our comprehensive program that provides access to providers and CBOs (e.g., informal caregiver support groups, medical condition education), integrated technology solutions, and a platform for caregiver training and certification. Within CaregiverConnect, we created Michigancaregiving.org for all HAP CareSource enrollees and their caregivers (both formal and informal) to provide caregiver support through user-centric content and easy-to-use interface for resources in the state of Michigan. These caregivers receive a direct and customized link offering personalized access to Michigancaregiving.org for the specific medical conditions in which they are caring.
