In Lieu of Services (ILOS)

As a part of Michigan Department of Health & Human Services’ (MDHHS’) continuous commitment to deliver Michigan residents equitable, coordinated and person-centered care, MDHHS encourages Michigan’s Medicaid Health Plans (MHPs) participating in the Comprehensive Health Care Program (CHCP) to offer ILOS that address Medicaid members’ health-related nutrition needs. ILOS are services the state deems to be medically appropriate and cost-effective when provided as substitutes for other Medicaid covered services.

This initiative will build on and complement the existing food and nutrition strategies in Michigan. MDHHS’ goal for ILOS is to improve health and reduce the future need for medical services by reducing food insecurity and ensuring enrollees have access to nutritious foods.
Members may qualify for one of these services at no cost. The food service HAP CareSource is offering is:

  • Medically Tailored Home Delivered Meals – members may choose to use a food service, if they qualify. The member’s Medicaid coverage and access to other medical services will stay the same if you use a food service or if they choose not to. Members can file a grievance or appeal about the food service, for example, if they are not approved for a food service. Information on how to file a grievance or appeal can be found within the member handbook
  • Medically Tailored Home Delivered Meal Through the Medically Tailored Home Delivered Meal Service – members will receive up to two healthy meals delivered to their home, seven days a week for up to six months. These meals are tailored to their health needs. Members will also get help from a registered dietitian. This person is a nutrition expert and will give you guidance on choosing healthy foods.

    This service is for members who:

    • Clinical Risk Factors
      • Have a nutrition-sensitive condition, including diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), hypertension, human immunodeficiency virus (HIV), cancer with malnutrition, sickle-cell disease, renal disease, gestational diabetes or other high-risk perinatal conditions.
      • Have been discharged from the hospital or a skilled nursing facility within the last 20 days.
    • Social Risk Factor
      • Is at risk for nutritional deficiency or nutritional imbalance due to food insecurity, defined as being unable to obtain nutritionally adequate, medically appropriate and/or safe foods.
