With locally based provider services, we will focus on our provider relationships and customer care. How to Join the CareSource Network
It is simple to start the process!

CareSource is a participating organization with CAQH. Please make sure that we have access to your provider application before you submit your CAQH number, as required on the Health Partner Contract Form:
- Log on to CAQH using your account information.
- Select the “Authorization” tab.
- Make sure CareSource Management Group is listed as an authorized health plan. Choose the Nevada location as the primary location, if applicable.
- If not, check the “Authorized” box to add.
On the Health Partner Contract Form, add the CAQH number under the “Provider Details” section.
