Important Changes Coming to CareSource MyCare Ohio

Starting January 1, 2026, CareSource will begin offering a combined Medicare and Medicaid plan, CareSource® MyCare Ohio (HMO D-SNP). As part of the transition to the Next Generation MyCare program, CareSource will no longer offer the CareSource® MyCare Ohio (Medicare-Medicaid Plan) as of December 31, 2025. Update Your Information

Changing office locations/phone numbers, adding/terming a provider, changing/adding specialties at a location, and/or closing a location are all reasons to notify CareSource.

Ensure your claims and prior authorization requests process without delay by reviewing and updating your information on file. From the left navigation menu, select Provider > Provider Maintenance for more information.
