To view your claims and Explanation of Benefits (EOBs), log into your CGHC Member Health Portal powered by CareSource account at My.CareSource.com. You can also access pharmacy benefits and wellness tools in the Member Health Portal.

To make a premium payment, set up recurring payments or update your information, log into your Pay My Premium Portal at CommonGroundHealthcare.org/Pay.

New Billing Payer Information
Providers have three options for submitting claims:
1. Electronic Data Interchange (EDI) – While Availity is our dedicated clearinghouse partner, any clearinghouse can be used to submit an EDI transaction using our Payer ID: MICS1.
2. HAP CareSource Provider Portal – Upload an image or use the direct entry.
3. Mail Paper Claims – Send paper forms to:
