Avoid Claim Rejections

Ohio Medicaid and MyCare Providers: Avoid Claim Rejections and Confirm Your Information Is Up-to-Date With ODM

If the claim is not an emergency claim, CareSource will reject it if any the following requirements are not met:

  1. The rendering provider on the claim is listed as an active provider in the Ohio Department of Medicaid (ODM) Provider Master File (PMF) as of the claim’s Date of Service.
  2. The billing provider on the claim is listed as an active provider in the ODM PMF as of the claim’s Date of Service.
  3. The rendering provider is affiliated with the billing provider on the ODM PMF as of the claim’s Date of Service.
