MyHealth Rewards

For adults 18 and older. Rewards are earned once each year unless noted. Rewards earned in the current year expire mid-December of the next year.

Rewards for Expectant Moms

First prenatal visit. Earn once each pregnancy.

Syphilis screening. Earn once each pregnancy.

Postpartum visit 7-84 days after the delivery date. Earn once each pregnancy.

Rewards for Preventive Care

Completion of the Health Needs Assessment in CareSource MyLife.

Breast cancer screening. For females age 40 and older.

Cervical cancer screening. For females ages 18 and older.

Completion of a tobacco cessation program.

Colorectal screening.

Rewards for Chronic Care

Retinal eye exam. Must have a diabetes diagnosis.

Kidney screening. Must have a diabetes diagnosis.

Rewards are subject to change. Rewards vary by age, gender, and health needs. If you are no longer with CareSource, access to your rewards will be deactivated. Any unused rewards may not be available.
