Care & Disease Management

CareSource offers care and disease management programs that can help you. These programs help you learn about your health and how you can manage your specific health conditions. Our goal is to make sure you have the right tools to stay as healthy as possible. These programs are available to you at no cost.

Care Management

CareSource has nurses and other outreach workers on staff who can work with you one-on-one to help coordinate your health care needs. They can contact you by phone:

• If your doctor requests one to contact you.
• If you request a phone call.
• If our care management staff feels their services would be helpful to you or your family.

Our staff is trained to help you with any special medical problems like asthma, cancer or other medical conditions. We can also work with you if you need help figuring out when to get medical care from your doctor, an urgent care center or the emergency room.

Disease Management

CareSource understands that living with a long-term condition is sometimes hard to manage. We offer free free disease management programs that can help you learn about your health and how you can better manage your specific health conditions. Our goal is to make sure you have the right tools to stay as healthy as possible. We help our members through:

• The MyHealth online tool for members 18+ to participate in a journey to improve their health.
• Materials with helpful tips and information to manage their condition and promote self-management skills.
• Care Coordination with outreach teams.
• One-to-one care management (if they qualify).

We may receive information from your doctor, pharmacy or other health care source letting us know that you would benefit from a program. We will send you materials related to your health condition along with tips and resources to help you self-manage your condition.

Each year, we review and update our information about health conditions from the leading national sources.

All ages (children, teens and adults) are eligible to participate in a program. Any member may self-refer or be referred into a disease management program to receive condition-specific information or outreach.
