This digital tool includes health assessments, guides, videos, and online tools to set and track health goals. It also offers information and support for nutrition, exercise, preventive care, emotional health, quitting tobacco or drugs, and limiting alcohol use. Access MyHealth through CareSource MyLife.

Use MyResources to find no- or low-cost resources for food, housing, school, financial support and more. Use the tool through CareSource MyLife.

Telehealth
Use your phone, tablet or computer to talk to a provider from wherever you are. There is no cost to you and there is no stress to find a ride. Learn more about telehealth.

Personal Emergency Response System (PERS)***
Get a personal emergency response system (PERS) device at no cost. PERS helps you call for help in an emergency, such as a fall at home. Push one button to reach a 24-hour call center. Ask your Care Coordinator if you qualify.
Meal Delivery***
After an inpatient hospital or skilled nursing stay, you may qualify for meal delivery through Mom’s Meals. You get two meals a day for 14 days after each stay. Ask your Care Coordinator if you qualify.
