Health Needs Assessment (HNA)

Our Health Needs Assessment (HNA) helps identify health needs. The HNA can be used to help make recommendations for your physical and mental health.

You can fill out the HNA:

  • Online: Login into your secure member portal account. Click on the Health tab.
  • Mail: If you are a new member, you get an HNA in your new member kit. Fill this out and return it in the postage paid reply envelope. Call Member Services to get another copy.
  • Phone: Member Assessment Team 1-833-230-2010 (TTY: 1-833-711-4711 or 711).
