Health Needs Assessment (HNA)

CareSource asks that all members complete an annual Health Needs Assessment (HNA). Through a few questions about your health and well-being, CareSource can help identify possible health issues, housing, education, and employment concerns and we may be able to provide resources to help address these concerns.

And, if you complete your assessment within your first 90-days of enrollment with CareSource $15 will be added to your My CareSource Rewards Gift Card. Go to My CareSource Rewards program for more information.

Members can complete the HNA by clicking on the Login logo at the top of this page. Once you have access to your My CareSource account click on the Health tab in the top navigation bar and scroll to the Health Needs Assessment section of the page.

My Health

When members age 18 and over they get a personalized health guide in their private My Health account. It will show how you can get healthy and lower your risk for health problems through:

  • Nutrition and exercise
  • Preventive health
  • Emotional health
  • Safety behaviors
  • Tobacco, drug and alcohol use
  • Blood pressure, cholesterol, body mass index (BMI)

You won’t have to wonder where to begin or what to do next — it’s there in your guide. Come back to the tool often to track your progress.
