Get Rewarded for Healthy Habits
Make life more rewarding and take charge of your health! Adults ages 18 through 64 years old may be able to earn rewards through the MyHealth Rewards program. You can redeem your rewards for gift cards to your favorite stores. You can find additional information about the MyHealth program and track your rewards from the Health tab in your My CareSource® member portal account.

Rewards are subject to change. The rewards available to you will vary depending on your health care needs. Please note that not all reward activities are covered services annually. You may be responsible for the cost if you do not check with CareSource or your primary care provider (PCP) before receiving services.
Redeeming your Rewards is Easy
- Complete one or more reward activities.
- Your provider’s office will submit a claim for the activity to CareSource
- CareSource will process the claim and add rewards points to your MyHealth Rewards account. Please allow 60 days for processing.
- Log in to your account to redeem you points for gift cards to popular retail stores.
