PLAN OPTIONS:

CARESOURCE SILVER
Our CareSource Silver plans are your best value if you qualify for tax credits and cost sharing. As the name implies, our Silver plan falls between our Gold and Bronze plans in both premiums and out-of-pocket costs. Benefit amounts vary based on income and plan level: Silver, Silver 1, Silver 2, or Silver 3. These plans include pediatric vision and dental (including orthodontia) for children up to the age of 19.

CARESOURCE SILVER DENTAL AND VISION
Our CareSource Silver Dental and Vision plan includes adult vision and dental. As with our other silver plans, your benefit amounts vary based on income and plan level: Silver, Silver 1, Silver 2, or Silver 3. You also have an optional fitness benefit.

CARESOURCE SILVER FEDERAL SIMPLE CHOICE
This plan allows easier comparisons across insurers by having generally consistent benefit levels. However, differences exist with the additional benefits, including provider networks, monthly premiums, and medications covered. This plan includes pediatric vision and dental (including orthodontia) for children up to the age of 19.

CARESOURCE SILVER FEDERAL SIMPLE CHOICE DENTAL AND VISION
This plan features the same benefits as CareSource Silver Federal Simple Choice but includes additional adult dental and vision coverage. You also have an optional fitness benefit.

CARESOURCE LOW PREMIUM SILVER
These CareSource plans have the lowest premiums of all of our Silver plans, but much higher deductibles. The plan includes pediatric vision and dental (including orthodontia) for children up to the age of 19.

CARESOURCE LOW PREMIUM SILVER DENTAL AND VISION
This plan features the same benefits as CareSource Low Premium Silver but includes additional adult dental and vision coverage. You also have an optional fitness benefit.

Evidence of Coverage

The Evidence of Coverage is an important legal document that describes the relationship between CareSource and plan members. It serves as your contract with CareSource and describes members’ rights, responsibilities and obligations. It also tells you how the plan works, the covered services you are entitled to, any conditions and limits related to covered services, the health care services that are not covered by the plan, and the annual deductible, copayments and coinsurance you must pay when you receive covered services.
