Care & Disease Management
Were you recently released from the hospital?
CareSource offers care management and disease management programs. These programs can help you learn about your health and how you can better manage your specific health conditions. Our goal is to make sure you have the right tools to stay as healthy as possible. These programs are available to you at no cost.
CareSource offers care management services that are available to children and adults. The care management team includes registered nurses, social workers, care guides, and community health workers. They are called care managers. Care managers work with you, your primary care provider (PCP) and/or other specialists, and any family or caregivers you would like to help coordinate your care. Together, we work with you to meet your health and wellness goals. You also have access to a collaborative meeting with your providers and other caregivers. Care managers can help schedule an Interdisciplinary Care Team meeting (ICT).
Care management includes different levels of care. The level of care you receive depends on your condition and how much help you need managing your healthcare needs. One of the easiest ways to get started is by completing your Health Risk Assessment (HRA). Using a few questions about your health and lifestyle, we can determine the level of care management you may need.
You can take the HRA in one of these ways:
- Phone: Call 1-833-230-2011 (TTY: 711) between 7 a.m. to 6 p.m., Monday – Friday.
- Online: Just log into your secure com account and click on the Health tab.
Care managers may ask you questions to learn more about your specific health conditions. This way, they can help you understand your condition and how to better manage your health. They’ll also help you access services and local resources. Care managers can connect you with resources you need like food, clothing, and housing. They can even help you coordinate transportation to get medical care.
You may hear from a care manager if:
- Your provider asks us;
- You or your caregiver asks us to contact you;
- We think we can help you based on your medical claims.

Disease Management
CareSource knows that living with a long-term condition can be hard to manage. We want you to have the right tools to stay healthy. If you are living with chronic health issues like diabetes, asthma, high blood pressure, or high cholesterol, our disease management programs are just for you.
Our free disease management programs can help you learn more about your health. They can also help you manage your health condition. You can choose to be part of a program or we may hear from your doctor, pharmacy, or other provider that you would benefit from this program. Disease Management for Kids and Teens
- Asthma Center: Asthma keeps more kids home from school than any other long-term health problem. Help your child handle the condition, stay healthy, and stay in school.
- Diabetes Center: Does your child have type 1 or type 2 diabetes? Get help to handle diabetes and keep your child healthy.
MyHealth
Adults age 18 and older can get health tools called Journeys, track your workouts, and use small step guides to help you live a healthy life.
