The Healthy Benefits+ debit card provides all members $287 per month to purchase qualifying over-the-counter (OTC), dental, vision and hearing items, services and accessories at eligible locations. Dental Benefits
Preventive care (i.e., exams, cleanings, x-rays and fluoride) and comprehensive dental (i.e., simple extractions, minor restorations, periodontics and other non-Medicare covered comprehensive dental services including extractions, crowns, implants, and dentures). Plan limit of *$5,000 towards supplemental oral exam, cleaning, fluoride treatments and dental implants.

Vision Benefits
No cost routine eye exam. Routine Exam, Eyeglasses are covered: Age 21-59 – 1 pair every 2 years; Age 60 years or older – 1 pair per year.

Coming home from an inpatient or skilled nursing stay? Our benefits include nutritionally balanced refrigerated meals, ready to heat and eat, delivered to your door. Get two meals per day for 14 days (max 28 per stay).
