CareSource® offers individualized care management and education about various health conditions. We help you learn about your health and how you can better manage your specific health conditions like high blood pressure or diabetes. Our goal is to make sure you have the right tools to stay as healthy as possible. Care Management and educational resources are available to you at no cost.
Your health and wellness are important to us at CareSource. One of the advantages of CareSource MyCare Ohio (Medicare-Medicaid Plan) is that every member is assigned a Care Manager. We provide personal care. We use a team approach with your needs at the center. Your Care Team is a group of people who can help you meet your goals for a healthy life. Your Care Team may include doctors, specialists, loved ones and community resources.
It’s important to have someone you can trust to help you stay healthy. Your care manager will help coordinate all your health care needs and assist with communications between the Care Team members. He/she will help you take the best care of yourself and understand the importance of your medication plan, lab work or a medical test. Your Care Manager will be an advocate for your goals and help you navigate the health care system. CareSource Care Management will coordinate your Medicare and Medicaid benefits so you can have peace of mind and focus on your well-being and healthy life.
Whether it’s finding a community resource, making health appointments or assessing next steps for a health goal, your Care Manager will lead the way. Together with your Care Team, he/she can oversee your needs and coordinate your care.
